چکیده: 

برق گرفتگی یکی از مباحث مهم و پیچیده در پزشکی قانونی و طب کار می باشد که ابعاد مختلف آن قابل بررسی و تامل می باشد، گذشته از موارد نقص عضوی و آسیبهای الکتریکی، تشخیص نحوه حدوث و تابلوی مرگ نیز چه از نظر حقوقی و چه از نظر پزشکی قانونی، دارای اهمیت بسزایی است، برای نیل به این هدف و بررسی ابعاد گوناگون برق گرفتگی، پس از استخراج کلیه موارد مرگ و میز ناشی از حوادث، در طول یک و نیم سال مذکور ارجاع شده به سازمان پزشکی قانونی، فاکتورهای دموگرافیک و همچنین آثار و علائم برق گرفتگی و نحوه حدوث مرگ در اجساد با اعلام فوت برق گرفتگی استنتاج گردید، که در آن بیشترین میزان برق گرفتگی در دهه دوم و سوم عمر و در ارتباط با حوادث مرتبط با شغل بوده و اکثریت موارد مرگ و میر، با آریتمی ایست قلبی متعاقب فیبریلاسیون بطنی و خطرناکترین حالت برق گرفتگی، منجر به قوت عبور جریان از دست راست و انتقال از میان پاها قلمداد گردید، مهم ترین مسئله در برق گرفتگی پیشگیری از وقوع آن است و این میسر نمی گردد، مگر آنکه آموزش همگانی و بالا بردن سطح آگاهیهای جامعه، و رعایت بهداشت کار، توسط کارگر و کارفرما و حاکم نمودن بهداشت صنعتی، در سرلوحه شاغلین و جامعه انسانی قرار گیرد.

Background: Electrical injuries are rarely happened but it makes more harmful lesions comparing to other thermal injuries. The aim of this study was to report electrical burned patients according to electrical voltage in Shahid Motahari Burn Center.Methods: This Routine data base study was performed on patients with electrical burns which were admitted to Shahid Motahari Burn Center from April 2010 to March 2012. Demographic and clinical data had gathered from medical records. Association between voltage and morbidity or mortality was evaluated used SPSS v. 16.Results: Mean total body surface area of 287 patients (283 (98.60%) male and 4 (1.4%) female) with mean age of 30±0.7 years was 13.56±0.76% (range 1-100). There were 203 patients (70.7%) with low and 84 patients (29.31%) with high voltage injury. There was significant association between voltage and place of injury (p=0.001).Conclusion: High voltage injuries constitute large number of electrical injuries which more of these injuries occurred outdoor and in workplace and need more hospitalization. High voltage injuries are related with more amputation, so people and authorities should pay more attention to such injuries.

Background: Burn is one of the most significant injuries in industrial and developing societies and is one of the most important traumas leading to hospitalization. The aim of this study was to identify the epidemiology, geographical distribution, and outcome of electric burns in Fars province and to present the distribution map. Methods: In this descriptive-analytical study, the study population involved all electrical burn victims admitted to Amir al-Momenin and Ghotbeddin Hospitals from 2008 to 2019 in Fars province in the south of Iran. Data were analyzed using SPSS software version 22. Results: Among a total of 246 patients, the average age was 30. 78 ±,11. 07. The highest frequency among educational levels was among under-diploma patients (38. 6%), and the majority were employed (87. 4%). Also, most of the patients were from urban areas (70. 3%). The majority of burn incidences occurred at the workplace (57. 7%). Also, among the high voltage patients, 25 patients (30. 9%) had an amputation, while among low voltage only 12 patients (16. 2%) had an amputation. Non-surgical treatment was applied in 68 (28%) cases, while Escharotomy was performed in 28 (11. 4%) patients. There was also a statistically significant association between burn voltage and amputation (P= 0. 039). Conclusion: Based on our report, the rate of electrical burn injuries in Iran is still high, which underlines the need for stronger efforts in effective prevention, such as better public education and the establishment of strict regulations regarding the distribution and use of electricity.

سابقه و هدف: یکی از تبعات جنگ (بمباران های هوایی) و حوادث غیر مترقبه مانند: زلزله، تصادفات رانندگی، بمب گذاری های تروریستی، لغزش زمین وریزش معدن، ریزش آوار و خرابی ساختمان ها است که از عوارض آن باقی ماندن افراد زیر آوار برای مدت کم و یا طولانی می باشد که باید با ساخت پناهگاه ها از ضایعات ناشی از بمباران ها کاست و در صورت وقوع آن امکانات خدمات رسانی، امدادی و درمان به سرعت در محل حاضر شده و درمان و نجات را شروع کنند. سندرم کمپارتمان و Crush syndrome مقوله هایی جدا هستند که گاه به اشتباه معادل هم به کار برده می شوند ولی Crush Injury می تواند هر دوی این سندرم ها را ایجاد کند. (شکل 1) در این مجموعه دو مقوله فوق مورد بحث و بررسی قرار گرفته و درمان های جدید و مقالات مختلف مرور شده است.

Introduction: Electrical burns are a less prevalent type of burn injury. However, some acute and chronic complications only occur in this type of burn due to its underlying mechanisms that lead to tissue injury. This study aimed to describe epidemiological patterns of electrical burns in the north of Iran to improve the prevention and management of burns in the area.Method: All medical records of patients who were admitted to Velayat Burn Center, Guilan University of Medical Sciences, Rasht, Iran, due to electrical burn injuries between March 2010 and March 2020 were studied retrospectively.Results: A total of 271 patients were included in this study. Occupational accidents (59.78%) were markedly more common than non-occupational injuries (40.22%). High voltage was responsible for 44.6% of the burns. Among the clinical outcomes studied, wound infection and the need for blood transfusion were the most frequent complications, with recorded rates of 17% and 18.5%, respectively. Regarding surgical procedures, limb amputations were performed in 8.1% of patients, 9.6% of cases required a flap, and 56.1% required a skin graft. The rate of transplant rejection was 2.2%.Conclusion: The higher incidence of occupational electrical burns in men of working age with non-academic education and the significant correlation between high voltage and worse clinical outcomes in this type of injury, compared to other types of burn injuries, indicate the need for employers to raise safety awareness in this area.

BACKGROUND Burns are one of the most important health problems in communities. Traumatic injuries, especially Traumatic Brain Injury (TBI) associated with burns, may increase disability and mortality. In addition to preventing burns, any action for a better treatment approach and early detection of concomitant traumatic injuries can reduce complications, disability, and treatment costs. We aimed to investigate the outcome of children with burn injury with and without TBI. METHODS In this cross-sectional study, 392 children with burn injuries treated at Motahari Hospital in Tehran, Iran from 2018-2019 were enrolled. Patient demographics, burn injury information and TBI-related information including head trauma and fracture were recorded in a checklist. Patients were divided into two groups of death (24 people) or discharge (368 people) in terms of outcome and the underlying variables were compared in the two groups. RESULTS There was no significant difference between the mean age of patients and gender in the two groups. The difference in the length of hospital stay, inhalation injury and skull fracture in the two groups was not statistically significant. The mean burn severity based on Total Body Surface Area (TBSA) and the frequency of TBI in the deceased group was significantly higher (P=0. 001). CONCLUSION The severity of burns based on TBSA and TBI is associated with increased mortality among children with burn injuries. The results suggest the need to examine children with burn injuries for TBI using clinical examination or imaging.

Case report: Severe burn injuries are associated with significant pathologic changes in nearly all of organ systems, among them are facial and airway injuries. Thus, one of the important points is airway management in head and facial burn injuries which lead to difficult or impossible tracheal intubation in these patients. In this study we introduce a 37 year-old man, who was burned with electricity and had contracture injuries in his face, hands, and circumorally. He was candidate for reconstructive lid surgery 84 days post-burn. Because of severe restriction in the mouth opening we intubated him Trans nasally with preservation of spontaneous breathing.In circumoral post burn injuries with restriction in mouth opening, it seems reasonable to intubate trachea with preservation of spontaneous breathing and extreme caution when using drugs; especially when flexible fiber optic bronchoscope is not available. In these situations, nasal tracheal intubation with head and neck movement by Glidescope could be useful as an alternative.
